Appointments can be scheduled through the office or can be made online by clicking here

Cancellation Policy
Failure to keep your scheduled appointment hinders our ability to provide the best care to our patients. Please call at least 24 hours in advance to cancel an appointment. Please note: There is a $25 charge for missed appointments and/or failure to cancel an appointment with less than 24 hours notice.


Insurance
Our office works with two forms of insurance:

  • Vision benefits that pay towards routine examinations and materials such as glasses and/or contact lenses
  • Major medical insurance, which is accepted for health-related visits and emergencies

We are currently in the process of becoming a preferred provider for many insurance companies.  Even if your insurance company is not listed below, please contact our office to find out if we can work with your insurance as an out-of-network provider. Many insurance plans will cover the bulk of an exam even out-of-network.

Billing & Payment Policies
All co-payments are due at the time of the appointment.  
We accept credit cards (MC, Visa, and Discover), checks, and cash.
